Provide Basic Patient Info

Refer a patient to ExactCare for support by entering the patient’s name, address and date of birth. You will also have the option to enter number of medications, insurance information and other notes about the patient.

Schedule the Assessment

If you want to schedule the patient’s assessment for them, you can immediately choose a day and time for the patient’s assessment and get instant confirmation of the appointment.

Review and Submit

After you refer or schedule an assessment, you will receive a summary of all of the information you input before submitting the information as well as immediate confirmation that we received the patient’s information.

Track Patient Status

Access real-time data on the status of all patients you refer and/or schedule including updates about their assessment, medication deliveries and more!

You can get patients started with ExactCare anytime from anywhere! Just download the ExactCare app to access your account on a cell phone or tablet. Visit the App Store or Google Play to get started.

Frequently Asked Qustions

Yes. Just click “Login” at the top of the screen next to the register button and input your username and password you currently use with the ExactCare app.
First, check your spam/junk folders for the email. If you still do not see the email, we may need to collaborate with your IT team to allow incoming emails sent from ExactCare. This will help ensure you receive future emails from ExactCare regarding your account along with other communications.
The ExactCare app is HIPAA compliant to ensure patient information is safe and secure.
The My Patients section includes all of the patients you have referred who have a particular status. By clicking into a status, you can view all of the patients with that status and more information. To see the full list of statuses and descriptions, click here.
You do not need to make an account to refer a patient. However, to utilize this method of referring, you must create an account. With an account, you will be able to refer and schedule assessments as well as view the statuses for all the patients you refer through the application. Without an account, you can refer patients via website form at www.exactcare.com or by calling 1-844-287-1609.
